1. Has your dog ever been to a off leash dog park or daycare? If so which one?

2. How is your dog when in an off-leash setting, are they high or low activity?

3. How often are they in an off-leash setting?

4. What does your pup look like when playing with another dog, do they wrestle, chase, bark, are they focused on one dog etc? What do they like to do?

5. Will they stop when asked?

6. How is your pup when they see squirrels, cats, bikes, bunnies etc.?

7. How is your dog if I take a bowl of food away? Are they food protective?

8. Has your dog ever bitten a person or other dog?

9. Has your dog ever had any training classes?

10. What is the reason you want your pup to attend dog daycare?

11. How is your pup in a kennel/crate?

From the time of your pups drop off until 2:15pm we will be evaluating your pup at our facility. Please plan ahead so that your pup can be properly evaluated for the duration of the daycare day.
